Support Coordination - Level 2

 

This support will assist you to build the skills you need to understand and use your plan. A Support Coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

Our Support Coordinators are experienced and committed to providing a person-centred and strengths-based approach. Your Wellspace Australia Support Coordinator will play an integral role in supporting you and your family navigate, activate, interpret and implement your NDIS Plan.

Specialist Support Coordination – Level 3

 

This is a higher level of Support Coordination. It is for people whose situations are more complex and who need specialist support. A Specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

Our Specialist Support Coordinators navigate and negotiating complex support plans, strive on identifying opportunities for innovative service delivery that empowers you to exercise choice and control in accessing a range of supports and services to overcome barriers in care of everyday life.
